Games and Supervised Practice should use a Parks and Recreation
Department form or their own computerized scheduling program form ¨ Tournament / Special Event USER should submit a bracket of games with the Park, dates, fields and times of each event Submit to Parks and Recreation Office a List of Leadership, Contact Information and Team/Player List for Placement in EXHIBIT of User Agreement¨
Standard User Agreement patrons must provide a list of all
leadership, addresses, phone numbers, e-mail addresses and title in
organizations and a roster of all players or ball field Agreements
may submit teams – coaches list. ¨ Tournament or Special Event USER must provide a list of all their leadership, titles or duties, contact information and a list of all sponsors. Submit to Parks and Recreation Office Proof of Liability and Injury Insurance to place in EXHIBIT of User Agreement¨ A letter of application for insurance may be temporarily approved by the City Manager and placed in the EXHIBIT until proof of insurance coverage is received. ¨
Documents providing proof of liability and injury insurance for
review by City Manager and placed as an EXHIBIT in the User Agreement ¨ With pre-approval by the City Manager, players and leadership may sign a Liability and Injury Insurance Wavier and incorporate them in the EXHIBIT of the User Agreement. User Fee Payment and Signing of the User Agreement¨

Make an appointment with the City Secretary in City Hall, ( All Changes to the User Agreement Shall be Approved by the City Manager and Any Needed Changes to the Official Schedule of Events in the EXHIBIT Should be Communicated and Approved by the Recreational Coordinator so that the Concessionaire and the Athletic Field Crews can modify their Work Schedules. Cancellation by the City of Sulphur Springs, Texas, for Weather or Unavoidable Conditions will be Rescheduled by the Parks and Recreation Department as Soon as an Acceptable Time Becomes Available
[*] The original Official Schedule of Events and any subsequence approved changes will be posted on www.colemanpark.com. The Official Schedule of Events as posted will be the guide for the Parks and Recreation Department to schedule, prepare and maintain the athletic fields. Contact the Recreational Coordinator if the Official Schedule of Events is not up-to-date or accurate. [†] Any changes to reduce the User Fee will not be refunded unless another team or group reserves the field and execute an User Agreement with the City of Sulphur Springs, Texas, for those same fields, dates and times [‡] . Added events may have an additional User Fee to be paid before the Events will be placed on the Official Schedule of Events and permitted to be played. Charges for incurred costs to operate on Holidays, Sundays and any Athletic Field Crew overtime may be added to the User Fee charge. |
